INTAKE/OUTREACH SPECIALIST

GENERAL DESCRIPTION OF DUTIES: The Intake/Outreach Specialist provides services in a fully integrated, holistic and collaborative manner using a team-treatment approach with JoH staff and community partnering agencies that ensures the coordination of all client services. This integrated model aims to facilitate clear communication amongst all providers and eliminates duplications of efforts or conflicting approaches to care and ensure wrap-around support. The Intake/Outreach Specialist provides a public face for Jobs of Hope. In terms of Intake, this position works with potential clients to explain the program features and requirements, as well as interview and assess clients for mutual fit within the program. The Intake/Outreach Specialist makes the final decision on admittance to the program, and also provides a complete needs assessment so that the Case Manager and Peer Counselor can begin developing goals with clients. As for Outreach, this position maintains close relationships with staff at various CO State prisons, as well as Weld County based Parole and Probation departments. Additionally, the Intake/Outreach Specialist supports Jobs of Hope at various community events and meetings. This position assists the Executive Director in maintaining a favorable public perception of the program. Supervision Received: Direct supervision provided by the Executive Director. Salary Range: $20.00-$25.00 (Compensation based on experience and education). This position can be either a full-time (40 hours per week) or part time (less than 30 hours per week) position. ESSENTIAL DUTIES AND RESPONSIBILITIES: •Coordinates prerelease reentry plans with potential participants via their in jail/prison case management teams, parole, probation, and other community partners •Accurately completes and processes all new participant intakes to include uploading documentation and entering data into the Homeless Management Information System (HMIS) database system. •Creates the initial client file and documentation. •Screens applicants via phone, video conferencing or in person to determine program eligibility and commitment to program expectations. •Communicates relevant resident information to staff on all new intakes •Administers intake urinanalysis and drug testing. •Maintains the House of Hope Transitional Housing waitlist. •Assists new participants in obtaining documentation for employment (ID, SS Card, Birth Certificate) •Assist participants with resumes, online applications, and interviewing skills. •Works with inmates’ transitional team to develop a comprehensive release plan. •Participates in required internal and external meetings. •Nurtures new and old relationships with donors, stakeholders and/or community partners. •Friendly outreach to individuals, groups and partnering agencies to promote agency services and collaboration to serve those in need of a successful reentry program. •Increases community knowledge of the Jobs of Hope program and enrollment. •Networks with community resources to identify referral mechanisms •Assists in the development of marketing materials •Assists in the development and implementation of a productive intake and outreach program. •Uses creativity and innovation in program development and service delivery. •Applies appropriate techniques and standards in program development and service delivery consistent with the mission and values of Jobs of Hope. •Implements program objectives and performance standards as directed by the Executive Director. OTHER DUTIES AND RESPONSIBILITIES: •In coordination with other staff, provide in-person coverage at the JoH office during office hours. •Flexibility to attend various inter-agency meetings or program events in the evenings or weekends as assigned by Executive Director. •Attends professional development and community trainings as approved by Executive Director. •Assists in the development and implementation of program policies and procedures. •Coordinates responsibilities with other staff members to insure an integrated team approach to all programs and day-to-day operations of JoH. •Any other tasks or duties as assigned. MINIMUM QUALIFICATIONS: Preference for an individual who has the strength, experience and passion to work with adult clients experiencing significant life challenges, such as incarceration, gang affiliation, substance abuse, mental health issues, and legal issues. REQUIRED SKILLS, KNOWLEDGE AND ABILITIES: •Cultural competency. •Knowledge of general non-profit principles, objectives, standards, and methods. •Familiarity with principles of trauma informed care. •Knowledge of community and government services deliveries. •Knowledge of motivational interviewing concepts and techniques preferred. •Skill in interviewing and conducting individual needs assessments. •Skills to problem solve and advocate. •Demonstrated strong communication, organizational and interpersonal skills. •Knowledge of the basic theories and practices of social work, interviewing and assessment techniques, and professional reports and documents preparation. •Skills in establishing and maintaining effective working relationships with clients, other employees, organizations, and the public are required. •Flexible and ability to multi-task. •Basic knowledge of Windows computing devices and common office applications such as Gmail, Google docs, Microsoft Word, Excel, and Powerpoint. •Inputs client information, and maintains client and program data, including confidentiality. •Knowledge of the service population’s cultural and socioeconomic characteristics and the appropriate techniques and practices for the client population. •Ability to interact effectively as a team member and independently with JoH staff, and with a diverse client base. •Demonstrated qualities of empathy, assertiveness, and organization; a high degree of integrity and responsibility. •Ability to read, understand and follow oral and written instruction. •Ability to work independently in a small office environment which includes office cleanup and maintenance. •Timely responsiveness to emails, calls and client requests. •Reliable transportation and provide proof of insurance. •Bilingual preferred but not essential EDUCATION AND FORMAL TRAINING: •Successful completions of an Associate Degree in the field of Psychology, Social Work, Human Services, Criminal Justice, Communications or related field. •Equivalent combination of education, training and experience which provides the required skills, knowledge, and ability for the position may be considered in lieu of a degree. •Experience serving vulnerable populations, including ex-offenders and homeless preferred. RE-ENTRY CASE MANAGER/NAVIGATOR

GENERAL DESCRIPTION OF DUTIES: The Full Time (40 hours/week) Re-Entry Case Manager/Navigator provides services in a fully integrated, holistic and collaborative manner using a team-treatment approach with JoH staff and community partnering agencies that ensures the coordination of all client services. This integrated model aims to facilitate clear communication amongst all providers and eliminates duplications of efforts or conflicting approaches to care and ensure wrap-around support. Communicates directly with clients to obtain historical information on financial status, employment, housing, family supports, legal issues and other potential client needs resulting in the development of a client focused holistic plan. The Re-Entry Case Manager/Navigator will provide a supportive environment to help individuals meet their goals toward self-sufficiency, teach skills to work toward the individual’s self-identified life goals (social, educational, employment, housing, and benefits), maintain frequent communication with the JoH Program team and Executive Director, providing updates on the progress being made toward reaching and/or meeting goals. This position will provide advanced one-on-one assistance to clients of Jobs of Hope regarding access to program services and an extensive coordination with partnering community agencies to ensure clients are successfully connected to resources that will help improve their current situation. Will develop immediate goals centered on attainment of vital documents, listing of all outstanding court cases, and listing of all outstanding fees and fines. •Provides a clear understanding of the commitment/expectations of the program to each participant. •Provides a comprehensive initial appointment assessment to identify strengths and barriers to successful outcomes through interviewing. •Assists clients in the development of a client focused holistic plan for success. •Provides supportive services per program guidelines to assist clients in reaching their established goals. •Establishes with each client a schedule for ongoing appointments and check-ins to meet their goals. •Establishes collaboration, communication and coordination among all current and future community partners’ involved in the implementation of the client’s individualized plan. •Assists clients in scheduling appointments, follow-ups, and referrals. •Attends client appointments with community resources as needed. •Assists with and provides transportation to and from appointments on an as-needed basis. •Serves as a client liaison to communicate client needs and gaps in services. •Attends organizational and community-based meetings with community partners as required to ensure appropriate service delivery. •Creates and manages client files and documentation which maintains accurate and up-to-date case records; documents client contacts and progress to ensure program compliance and progress of goals. •Conducts crisis intervention and conflict resolution as needed and consult with Executive Director on serious conflicts that can’t be mediated. •Conducts exit evaluations with clients to measure outcomes achieved. RESIDENT HOUSE SUPERVISOR

GENERAL DESCRIPTION OF DUTIES: The Resident House Supervisor is a live-in position. Monthly rent and meal supplies will be provided in exchange for supervisor activities as specified in this document. The Resident House Supervisor coordinates with the Executive Director in the daily operations of the Rick Hartman House of Hope. The Resident House Supervisor will be responsible for working with the JoH Operations Manager to coordinate maintenance work and facility cleanliness, enforcing property rules, and handling tenant relations. Supervision Received Direct Supervision provided by the Executive Director, with support from the JoH Operations Manager. ESSENTIAL DUTIES AND RESPONSIBILITIES: Responsibilities include, but are not limited to the following: •Upholds all Jobs of Hope policies and procedures. •Oversees client compliance with policies and procedures. •Tenant satisfaction and addressing complaints. •Inspects facility for maintenance and cleanliness. •Provides facility maintenance and coordination of repairs and services, in cooperation with the JoH Operations Manager. •Coordinates the process of facility cleanliness, upkeep, safety and janitorial needs. •Assists JoH Operations Manager with cleaning and preparing rooms for new rentals. •Ultimate responsibility for clean and sanitary conditions at the Property OTHER DUTIES AND RESPONSIBILITIES •Provides after-hours and overnight response for tenant needs and emergency maintenance issues. •Coordinates with Executive Director to provide coverage during requested time off/vacations. •Coordinates with other staff members to ensure an integrated team approach. ETHICAL STANDARDS The position of Resident House Supervisor requires interaction with clients, and must display the highest degree of personal and professional integrity. It requires sensitivity to the religious, cultural, and socioeconomic issues of those we serve. The principles of equality, integrity, confidentiality, and privacy are to be always maintained. All clients are to be treated with dignity and respect. HOURS OF EMPLOYMENT The Resident House Supervisor will work approx 30 hours per week. These working hours will typically be after 5pm, up until curfew. 1 day off is provided per week. The Resident House Supervisor must have the ability to work flexible hours on evenings and weekends as determined by the JoH Operations Manager or Executive Director. QUALIFICATIONS REQUIRED •Basic handyman skills as needed. •Experience with power tools and knowledge of their safe operations. •Ability to treat disenfranchised and formerly incarcerated persons with respect and dignity. Demonstrated qualities of empathy, assertiveness and organization, a high degree of integrity and responsibility. •Must work well with tenants and other co-workers. •Must balance qualities of assertiveness and empathy. •Must have knowledge and familiarity with trauma-informed care. •Ability to read, understand and follow oral and written instruction. •Ability to communicate clearly. •Ability to problem solve in order to find effective solutions for a variety of potential situations. •Must have ability to use Microsoft Office programs including Word and Excel. Strong organizational and time management skills to multitask and prioritize efficiently. •Must display excellent customer service and interpersonal skills. •Bilingual preferred but not required.
